HOW IT WORKS
- STEP 1 Take 3-5 minutes to record your vital signs.
- STEP 2 The monitor transmits the recorded vitals to our Central Station.
- STEP 3 STRUCTURED TELEPHONE SUPPORT Elite TeleHealth will make recommendations based on the vitals transmission. If there is an alert, our nurse will call the client and/or family member or caregiver to provide recommendations on actions to take to remain within healthy parameters.
- STEP 4 Elite TeleHealth will provide track and trend reports to the medical home.
WHY IT WORKS
Elite TeleHealth prevents illness, symptoms and events that require medical attention through tracking and trending our client’s health.
With structured telephone support, we call the client and/or family member or caregiver to provide recommendations for immediate action. Whether it be medications, diet change, or medical intervention, we can change the course of an illness or medical situation.
If a Congestive Heart Failure client gains 2lbs. of fluid on Monday and 2lbs. on Tuesday, it becomes very dangerous. When the client lies down, fluid surrounds the heart, forcing it to work harder.
In this example, we would have known about the fluid weight gain on Monday and would have acted immediately to keep the situation from escalating.
Symptoms like shortness of breath, elevated blood pressure, and fatigue can present and, in some cases, even cause death. In most cases, we can detect these symptoms and prevent negative trends from occurring.
